Case Management
Case Management is the foundation for all services delivered or coordinated by Ability Beyond Barriers, formerly Horry County Disabilities and Special Needs. Ability Beyond Barriers has been providing exceptional case management services since 1984. We strive to assist families in identifying the supports and services they need utilizing the most person centered planning process. Once service needs are identified, the case manager will work directly with the individual and the family in addressing those needs thru referrals and linking to additional services.

Everyone, over the age of 3, is assigned a Case Manager to assist in linking families and consumers with services. For specific eligibility requirements please visit the South Carolina Department of Disabilities and Special Needs state website:
To begin the eligibility process you must call the referral number that best meets your needs:
Children 0-3:
Individuals with Intellectual Disabilities or Autism:
Individuals with Head or Spinal Cord Injuries:

You will be given a choice of Case Management providers with Ability Beyond Barriers, formerly Horry County Disabilities and Special Needs Board being one of them. We hope that you will select Ability Beyond Barriers to serve you and your family member. Once you have selected your provider, a Case Manager will be assigned.
