Do I Qualify?
To begin the eligibility process you must call the referral number that best meets your needs:
Children 0-3:
Individuals with Intellectual Disabilities or Autism:
Individuals with Head or Spinal Cord Injuries:
Spanish Speaking Families:
Please note: These numbers will be a recording and someone will call you back.
You will be given a choice of Case Management providers with Horry County Disabilities and Special Needs Board being one of them. We hope that you will select Horry County Disabilities to serve you and your family member. Once you have selected your provider, a Case Manager will be assigned.
Unfortunately, once eligible for services, individuals are placed on extensive waiting lists.
The Case Manager continues to play a key role in assisting the individual, however, funding is often an issue that blocks Case Managers from connecting individuals with all the services they need.
In preparation for your initial visit with your Case Manager, it would be helpful if you had the following information on hand. (Note: Not all items will be applicable for everyone). If you do not have access to a copier, we can copy these documents in our office and return the originals to you:
- All psychological evaluations
- School records (IEP)
- Other agency reports (Mental Health, previous programs attended)
- Medical reports
- Therapy reports (Speech, OT, PT)
- Copy of Birth Certificate
- Copy of Social Security Card
- Copy of Medicaid and/or Medicare cards
- Copy of Private Insurance card
Names, complete addresses and telephone number of past/present:
- Doctors
- Dentists
- Schools Attended
- Other Special Needs programs
